Job Description

A Registered Nurse (RN) with expertise in home health nursing is sought for a contract role providing vital in-home patient care. This position requires proficiency in OASIS assessments and a commitment to delivering personalized care plans that enhance patient outcomes.

Key Responsibilities:

  • Conduct comprehensive nursing assessments including vital signs, wound care, and medication management.
  • Develop and implement individualized care plans tailored to each patients needs.
  • Educate patients and families on disease management, treatment plans, and preventive measures.
  • Collaborate closely with physicians, therapists, and other healthcare professionals to maintain cohesive care.
  • Accurately document all patient care activities using electronic medical records (EMR).
  • Monitor patient conditions and promptly report any changes to supervising physicians.
  • Ensure adherence to all relevant federal and state regulations governing home health care.

Qualifications:

  • Active Registered Nurse (RN) license.
  • Minimum of 3 months home health experience with OASIS documentation.
  • Experience with Home Care Homebase (HCHB) software is highly desirable.
  • Strong clinical skills, including IV therapy, wound care, and medication administration.
  • Proficient in electronic medical records (EMR) systems.
  • Excellent communication and interpersonal capabilities.
  • Ability to work independently and manage time effectively.
  • Valid drivers license and reliable transportation.
