A Registered Nurse (RN) with expertise in home health nursing is sought for a contract role providing vital in-home patient care. This position requires proficiency in OASIS assessments and a commitment to delivering personalized care plans that enhance patient outcomes.
Key Responsibilities:
- Conduct comprehensive nursing assessments including vital signs, wound care, and medication management.
- Develop and implement individualized care plans tailored to each patients needs.
- Educate patients and families on disease management, treatment plans, and preventive measures.
- Collaborate closely with physicians, therapists, and other healthcare professionals to maintain cohesive care.
- Accurately document all patient care activities using electronic medical records (EMR).
- Monitor patient conditions and promptly report any changes to supervising physicians.
- Ensure adherence to all relevant federal and state regulations governing home health care.
Qualifications:
- Active Registered Nurse (RN) license.
- Minimum of 3 months home health experience with OASIS documentation.
- Experience with Home Care Homebase (HCHB) software is highly desirable.
- Strong clinical skills, including IV therapy, wound care, and medication administration.
- Proficient in electronic medical records (EMR) systems.
- Excellent communication and interpersonal capabilities.
- Ability to work independently and manage time effectively.
- Valid drivers license and reliable transportation.
