This position offers the opportunity to work with an experienced care team, providing compassionate support to home health patients while utilizing your clinical expertise and OASIS documentation skills.
Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be required
Key Responsibilities:
- Deliver skilled in-home nursing care, including assessments, disease monitoring, and medication management
- Document patient care using OASIS with precision and timeliness
- Develop, update, and execute individualized care plans in collaboration with physicians and healthcare partners
- Perform wound care, patient education, and other skilled nursing interventions
- Maintain clear communication with care teams regarding patient status and updates
Qualifications:
- Active RN license required
- Minimum of one year of recent experience in home health nursing
- Proficient in OASIS documentation (required)
- Experience with HomeCare HomeBase (strongly preferred)
- Reliable transportation and a valid drivers license
- Strong organizational and time management abilities

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
