An opportunity is available for a Registered Nurse (RN) with home health expertise to provide compassionate, patient-centered care on a contract basis. This role focuses on delivering high-quality nursing services in patients' home environments, ensuring comfort and personalized attention.
Key Responsibilities:
- Conduct thorough initial and ongoing patient assessments, including vital signs monitoring, medication management, and wound care.
- Develop and implement individualized care plans tailored to each patients unique health needs.
- Educate patients and their families on disease management, treatment regimens, and preventive health strategies.
- Collaborate closely with physicians, therapists, and other healthcare professionals to coordinate comprehensive care.
- Accurately document all nursing interventions and patient progress in electronic medical records (EMR).
- Monitor patient conditions vigilantly, reporting any significant changes to the supervising physician in a timely manner.
- Adhere strictly to all state and federal regulations governing home health care services.
Qualifications:
- Active Registered Nurse (RN) license.
- Minimum of 3 months of home health nursing experience with proficiency in OASIS documentation.
- Experience with Homecare Homebase (HCHB) software is highly advantageous.
- Strong clinical capabilities, including IV therapy, wound care, and medication administration.
- Proficient in EMR systems for accurate patient documentation.
- Excellent communication skills and ability to work independently.
- Valid drivers license and reliable transportation required for home visits.
