This 13-week assignment involves delivering high-quality, compassionate care within a field-based home health setting, utilizing your expertise to foster better health outcomes for each patient.
Schedule & Work Environment:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Field-based visits to patient homes (mileage reimbursement provided)
- Average caseload: 35 patient visits daily (30 points per week)
- Electronic documentation via Home Care Home Base (HCHB)
Key Qualifications:
- Current, active RN license in good standing within the state of practice
- Minimum of one year of recent home health nursing experience (field work strongly preferred)
- Demonstrated proficiency with Home Care Home Base (HCHB)
- OASIS assessment experience required
- Reliable transportation, drivers license, and auto insurance
Primary Responsibilities:
- Complete comprehensive OASIS assessments and develop individualized care plans
- Deliver skilled nursing care, including medication administration, wound care, IV therapy, and chronic disease management
- Monitor and document patient progress, noting changes in condition and communicating promptly with the interdisciplinary team
- Educate patients and families on treatment plans, medications, and preventative strategies
- Ensure timely, accurate documentation in accordance with regulatory and organizational standards
- Collaborate with physical therapists, occupational therapists, social workers, and home health aides for coordinated care
- Participate in case conferences, staff meetings, and continuing education

$1,815 - $1,920/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
