Advance your nursing career with a dynamic opportunity supporting patients in their homes on a full-time, 13-week contract assignment. This role is integral in delivering compassionate, skilled care and helping patients achieve their health goals within the comfort of their residence.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based, traveling daily to patient homes within the assigned territory
• Typical caseload: 30 points per week (about 35 patient visits per day)

Qualifications:

• Current and active RN license in good standing, in the state of practice
• Minimum of 1 year of recent home health experience, with direct field-based patient interactions preferred
• Proficiency in Home Care Home Base (HCHB) is required
• Experience with OASIS assessment and documentation is essential
• Reliable transportation, a valid drivers license, and active auto insurance

Responsibilities:

• Complete comprehensive OASIS assessments and develop personalized care plans for patients in the home health setting
• Deliver skilled nursing services, such as wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient progress, promptly identify and communicate changes in condition to the physician and care team
• Educate patients and their families on disease management, medications, and preventive healthcare
• Maintain meticulous, timely documentation in HCHB
• Collaborate with a multidisciplinary home health team to coordinate holistic care
• Engage in case conferences, staff meetings, and ongoing professional education
• Comply with all regulatory and agency standards for both documentation and direct patient care

$1,815 - $1,920/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
