We are seeking a dedicated Registered Nurse with strong Home Health and OASIS experience for a 13-week full-time contract. This role offers a rewarding chance to collaborate with a professional, supportive care team and provide critical nursing services in a one-on-one setting.
Schedule:
- 13-week contract with potential for extension
- Full-time, Monday through Friday
- Daily structured visits
- On-call hours may be required
Key Responsibilities:
- Deliver comprehensive in-home nursing care, including skilled interventions, wound care, and medication management
- Accurately complete patient assessments using OASIS and submit timely documentation
- Partner with physicians and interdisciplinary teams to develop individualized care plans
- Educate patients and families on disease processes, medications, and ongoing care needs
- Monitor and communicate changes in patient condition to the clinical team
- Maintain accountability for daily visit schedules
Qualifications:
- Active RN license
- Minimum of one year recent home health RN experience
- Demonstrated OASIS proficiency
- Strongly preferred: experience with HomeCare HomeBase
- Ability to independently manage a structured, full-time patient schedule
- Reliable transportation and valid drivers license
