A dedicated Registered Nurse is needed for a 13-week full-time contract in home health, offering an engaging role for those experienced with OASIS and managing daily visit schedules. Schedule:
- Full-time hours, Monday through Friday
- On-call likely included
- 13-week contract, with possible extension
Key Responsibilities:
- Deliver skilled in-home nursing care and clinical interventions
- Conduct assessments and complete OASIS documentation accurately and promptly
- Collaborate with healthcare providers and interdisciplinary teams to coordinate personalized care plans
- Perform wound care, manage medications, provide patient education, and monitor chronic conditions
- Communicate changes in patient status with the clinical team efficiently
Preferred Experience and Qualifications:
- Current, active RN license
- At least 1 year of recent home health RN experience
- Required OASIS proficiency
- Experience with HomeCare HomeBase (strongly preferred)
- Confident managing a structured, daily patient visit schedule
- Reliable transportation and a valid drivers license
