Your next nursing opportunity is herea full-time, 13-week Registered Nurse role specializing in home health. Schedule:

• 13-week contract with extension potential
• Monday through Friday, full-time hours
• On-call responsibilities may be required
• Daily visit expectations set and structured

Core Responsibilities:

• Deliver skilled nursing care and interventions in patients homes
• Conduct thorough assessments and complete OASIS documentation promptly and accurately
• Collaborate with physicians and care team members on individualized plans of care
• Provide wound care, medication administration, patient education, and disease management
• Monitor and report changes in patient conditions to the larger clinical team

Qualifications:

• Active Registered Nurse (RN) license
• At least one year of recent home health RN experience
• Proficiency in OASIS documentation is essential
• Strongly prefer experience with HomeCare HomeBase EMR
• Must be able to efficiently manage a full-time caseload and daily visits
• Reliable transportation and a valid drivers license are required
