Schedule:
- Full-time, Monday through Friday
- On-call duties are probable
- Daily structured patient visit expectations
Qualifications:
- Active RN license
- At least 1 year of recent home health RN experience
- Demonstrated proficiency with OASIS required
- Experience with HomeCare HomeBase is strongly preferred
- Capable of managing a structured, full-time caseload
- Reliable transportation and a valid drivers license
Key Responsibilities:
- Provide skilled nursing care and interventions to patients in their homes
- Conduct comprehensive patient assessments and accurately complete OASIS documentation in a timely manner
- Collaborate with physicians, therapists, and the wider care team to create and implement individualized care plans
- Deliver wound care, medication management, disease monitoring, and patient/family education
- Maintain clear communication regarding patient status and updates with the clinical team
