Schedule:

• Monday to Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Active RN license in the state of practice and in good standing
• At least one year of recent home health nursing experience (field-based experience highly valued)
• Proficiency in the Home Care Home Base (HCHB) electronic documentation system
• Strong working knowledge of OASIS assessment tool
• Reliable transportation, valid drivers license, and current auto insurance

Core Responsibilities:

• Complete comprehensive OASIS assessments and design individualized care plans for each patient
• Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient progress, identify changes in health status, and communicate findings promptly to the healthcare team and physicians
• Educate patients and families on managing illnesses, medications, and preventive strategies
• Ensure timely and precise documentation in HCHB
• Collaborate with an interdisciplinary teamincluding therapists, social workers, and aidesto provide fully coordinated care
• Actively take part in case conferences, staff meetings, and continuing education
• Maintain compliance with all regulatory and agency standards

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
