Deliver high-quality, patient-centered nursing care in a field-based home health setting as part of a dedicated healthcare team. This full-time, 13-week contract assignment offers the chance to positively impact patients lives through comprehensive assessment, skilled interventions, and ongoing education, all within their own homes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Role Requirements:

• Active RN license in good standing for the state of practice
• At least 1 year of recent home health nursing experience (field-based strongly preferred)
• Proven proficiency using Home Care Home Base (HCHB) electronic documentation system
• Strong background in OASIS assessments and related documentation
• Reliable transportation, current drivers license, and valid auto insurance

Key Responsibilities:

• Conduct thorough OASIS assessments and develop individualized care plans for each patient
• Deliver skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient progress, assess for changes in condition, and communicate promptly with care team members and physicians
• Provide education and support to patients and their families regarding disease processes, medications, and preventive health measures
• Maintain precise, timely documentation within HCHB
• Collaborate closely with physical therapists, occupational therapists, social workers, and home health aides to foster coordinated, holistic care
• Engage in regular case conferences, staff meetings, and ongoing training as needed
• Consistently adhere to regulatory and agency standards for both patient care and documentation

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
