Advance your nursing career by delivering compassionate care in a dynamic home health environment. In this contract role, youll work directly with patients in their homes, supporting recovery and long-term wellness while collaborating with an interdisciplinary care team.

Schedule and Setting:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Field-based, with daily travel to patient homes within your assigned territory (mileage reimbursement provided)
• Caseload: 30 points per week (generally 35 patient visits per day)

Key Responsibilities:

• Conduct OASIS assessments and develop individualized care plans
• Deliver skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management
• Monitor changes in patients conditions and communicate timely updates to the care team and physicians
• Educate patients and their families on treatment plans, medications, and preventive care strategies
• Document visits and interventions accurately and efficiently using Home Care Home Base (HCHB)
• Collaborate with physical therapists, occupational therapists, social workers, and home health aides for comprehensive patient care
• Attend case conferences, staff meetings, and participate in ongoing professional development
• Maintain compliance with regulatory and agency care standards

Qualifications:

• Active RN license in the state of practice, in good standing
• At least 1 year of recent home health nursing experience (field experience highly preferred)
• Hands-on experience with Home Care Home Base (HCHB) and OASIS required
• Reliable transportation, valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
