Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Current, active RN license in good standing for the state of practice
• At least 1 year of recent home health nursing experience (field experience strongly preferred)
• Proficient with Home Care Home Base (HCHB) documentation system
• Hands-on experience with OASIS assessments
• Valid drivers license, reliable transportation, and current auto insurance

Responsibilities:

• Conduct comprehensive patient assessments using OASIS and develop individualized care plans
• Deliver skilled nursing care, such as wound and medication management, IV therapy, and chronic disease education
• Monitor and evaluate patient progress, reporting any changes in condition
• Provide thorough patient and family education on disease management, treatments, and medication regimens
• Maintain timely, accurate documentation in HCHB
• Work closely with multidisciplinary team members, including therapists, social workers, and aides, to ensure coordinated services
• Participate in regular case conferences, staff meetings, and seek continuing education as needed
• Uphold regulatory and agency care standards

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
