We are seeking a dedicated Registered Nurse (RN) with proven home health and OASIS experience for a 13-week full-time contract. If you thrive in the home health setting, excel at accurate documentation, and possess exceptional clinical skills, this role offers an ideal platform to make a real difference in patient lives.
Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be required
- Daily visit expectations structured for optimal patient care
Key Responsibilities:
- Deliver skilled nursing care and interventions directly in patient homes
- Complete comprehensive OASIS assessments and maintain thorough, timely documentation
- Work collaboratively with physicians and multidisciplinary care teams to implement individualized care plans
- Provide wound care, medication management, disease monitoring, and patient/family education
- Communicate patient status updates and changes to the broader clinical team
- Travel between patient homes within your assigned territory
Desired Qualifications:
- Active RN license in good standing
- At least one year of recent RN experience in home health
- Demonstrated proficiency with OASIS documentation
- Strongly prefer experience with HomeCare HomeBase software
- Ability to independently manage a structured patient schedule
- Reliable transportation and current drivers license required
