Job Description

Excel in a dynamic home health environment by providing compassionate, clinically skilled nursing care directly to patients in the comfort of their homes. This 13-week full-time contract position offers the opportunity to work Monday through Friday, 8:00 AM to 5:00 PM, managing an average caseload of 30 points per week (typically 35 in-home patient visits daily). Youll be responsible for both clinical interventions and meaningful patient education in a field-based role.

Key Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled interventions, including wound management, medication administration, IV therapy, and chronic disease monitoring.
• Monitor patient progress, respond quickly to changes in status, and communicate effectively with physicians and the care team.
• Educate patients and families on diagnoses, treatments, and preventive measures.
• Maintain detailed, timely documentation using Home Care Home Base (HCHB).
• Collaborate with PT, OT, MSW, and home health aides to ensure continuity and quality of care.
• Attend regular case conferences, staff meetings, and participate in ongoing education.
• Comply with all regulations and clinical standards for home health care.

Required Qualifications:

• Current RN license (in good standing) in state of practice.
• At least one year of recent field-based Home Health nursing experience.
• Demonstrated proficiency in Home Care Home Base (HCHB) and OASIS.
• Reliable transportation with a valid drivers license and required insurance.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
