Your next nursing opportunity is herea rewarding role for an experienced Registered Nurse specializing in home health. This is a full-time, 13-week contract with a chance for extension, offering a positive, collaborative work environment.
Schedule:
- Full-time position, Monday to Friday
- Daily structured patient visits
- Some on-call responsibilities may apply
Key Qualifications:
- Active RN license
- At least 1 year of recent home health RN experience
- Strong OASIS documentation skills (required)
- Proficiency in HomeCare HomeBase (strongly preferred)
- Reliable transportation and valid drivers license
- Excellent communication, time management, and clinical assessment abilities
Primary Responsibilities:
- Deliver skilled, in-home nursing care, including interventions such as wound care, medication management, patient education, and chronic disease monitoring
- Conduct thorough patient assessments and complete OASIS documentation accurately within required timeframes
- Collaborate with physicians and interdisciplinary care teams to ensure individualized care plans
- Monitor and report changes in patient condition promptly to the clinical team
- Maintain accurate records and deliver updates to ensure continuity of care
