Make a meaningful impact working directly with patients in their homes through this field-based role focused on quality care and fostering independence. This full-time, 13-week contract involves supporting patients Monday through Friday, 8:00 AM 5:00 PM, aiming for a caseload of 30 points weekly, commonly achieved with 35 patient visits per day. As part of the care team, youll be responsible for traveling within your assigned territory, providing hands-on support, and maintaining crucial documentation in Home Care Home Base (HCHB) using the OASIS assessment tool.

• Monday Friday, 8:00 AM 5:00 PM

Qualifications:

• Active RN license (in good standing)
• At least 1 year of recent field-based Home Health experience
• Proficient in Home Care Home Base (HCHB)
• OASIS documentation experience
• Reliable transportation, valid drivers license, and current auto insurance

Core Responsibilities:

• Complete OASIS assessments and develop individualized care plans for each patient
• Deliver skilled nursing interventions, including wound care, IV therapy, medication administration, and management of chronic conditions
• Routinely evaluate patient status and communicate significant changes with the broader care team and physicians
• Educate patients and families on disease processes, treatments, medications, and procedures
• Maintain precise, timely electronic documentation in HCHB
• Collaborate with a multidisciplinary teamincluding therapists, social workers, and aidesto promote seamless care
• Participate in team meetings, training, and continuing education
• Ensure compliance with regulatory and agency requirements

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
