This opportunity is ideal for an experienced Registered Nurse (RN) with a strong background in home health nursing and OASIS documentation. The 13-week full-time contract offers a structured Monday through Friday schedule with probable on-call duties, allowing you to deliver skilled, patient-centered care in the home setting.

Key Responsibilities:

  • Deliver comprehensive in-home nursing care and skilled interventions
  • Perform patient assessments and complete accurate OASIS documentation on schedule
  • Collaborate with physicians and multidisciplinary care teams to develop and implement individualized care plans
  • Provide wound care, medication management, patient education, and ongoing disease monitoring
  • Communicate patient status updates and any changes in condition promptly to the clinical team

Qualifications and Desired Experience:

  • Active RN license with minimum 1 year of recent home health experience
  • Demonstrated proficiency in OASIS documentation
  • Experience using HomeCare HomeBase software is strongly preferred
  • Ability to maintain a structured full-time schedule with daily patient visits
  • Reliable transportation and a valid drivers license
