A rewarding position is available for an experienced Registered Nurse seeking a full-time contract in home health. This 13-week assignment offers the chance to deliver impactful care to patients in their homes while working alongside a dedicated and professional clinical team. Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be required
Key Responsibilities:
- Deliver in-home nursing care and skilled interventions based on individualized patient needs
- Conduct assessments and ensure timely, accurate completion of OASIS documentation
- Collaborate closely with physicians and interdisciplinary teams to implement care plans
- Provide wound care, medication management, patient and family education, and chronic disease monitoring
- Communicate promptly regarding patient status changes to the care team
Qualifications:
- Active RN license
- At least 1 year of recent home health RN experience
- Required experience with OASIS documentation
- Experience with HomeCare HomeBase is strongly preferred
- Reliable transportation and valid drivers license
- Excellent clinical skills and ability to independently manage daily visit schedules

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
