Your next nursing opportunity starts here. We are seeking an experienced Registered Nurse with a background in home health and OASIS documentation for a full-time, 13-week contract assignment. In this role, youll contribute to meaningful patient outcomes and work alongside a skilled, compassionate care team.
Schedule:
- Full-time, Monday through Friday
- On-call rotation may be required
Key Responsibilities:
- Deliver skilled in-home nursing care and interventions according to individual patient needs
- Conduct patient assessments and ensure OASIS documentation is thorough and timely
- Collaborate with physicians and interdisciplinary team to develop and implement comprehensive care plans
- Perform wound care, manage medications, educate patients and families, and monitor disease progression
- Communicate promptly with the clinical team regarding changes in patient conditions
Qualifications:
- Active RN license
- At least one year of recent experience in home health nursing
- Proven OASIS documentation experience
- Proficiency with HomeCare HomeBase is strongly preferred
- Demonstrated ability to manage daily patient visits and a structured schedule
- Reliable transportation and a valid drivers license
