Embrace the rewarding challenge of supporting patients in their homes with a full-time, 13-week contract opportunity for an experienced Registered Nurse specializing in Home Health. This field-based position provides meaningful, hands-on care in Gresham, OR and plays a pivotal role in delivering personalized healthcare where it matters most.

Schedule/Hours:

• Monday Friday, 8:00 AM 5:00 PM

General Expectations:

• Caseload of approximately 30 points per week (usually 35 patient visits per day)
• Daily travel within your assigned territory (mileage reimbursement provided)

Key Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing care, such as wound care, IV therapy, medication administration, and chronic disease management
• Evaluate patient progress, monitor for changes in condition, and communicate promptly with the interdisciplinary care team and physicians
• Educate patients and their families on diagnoses, treatments, medications, and prevention
• Maintain meticulous, timely documentation using Home Care Home Base (HCHB) software
• Collaborate with therapists, social workers, and aides to ensure holistic care
• Engage in case conferences, staff meetings, and relevant continuing education
• Comply with all agency and regulatory guidelines for documentation and patient care

Qualifications:

• Active RN license in good standing for the state of practice
• At least 1 year of recent Home Health field experience
• Proficiency with Home Care Home Base (HCHB) and OASIS assessment tool
• Reliable transportation, drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
