Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Active RN license in good standing for the state
• At least one year of recent experience in Home Health (field experience preferred)
• Skilled in using Home Care Home Base (HCHB) electronic documentation
• Proficient in OASIS assessments and care planning
• Reliable transportation, valid drivers license, and current auto insurance

Core Responsibilities:

• Conduct comprehensive OASIS assessments to develop individualized care plans
• Deliver skilled nursing care including wound care, medication administration, IV therapy, and management of chronic conditions
• Monitor and report patient progress, including changes in condition, to the interdisciplinary team and physicians
• Educate patients and family members on diagnoses, treatment regimens, and preventive care strategies
• Document visits and updates efficiently within Home Care Home Base (HCHB)
• Collaborate closely with therapists, social workers, and home health aides for seamless care coordination
• Attend case conferences, team meetings, and relevant training sessions
• Ensure adherence to agency and regulatory standards

$1,742 - $1,843/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
