A full-time Registered Nurse (RN) with strong Home Health and OASIS experience is needed for a 13-week contract assignment. This is an excellent chance to deliver quality in-home care, utilize your documentation skills, and thrive in a supportive environment.
Schedule:
- 13-week contract, full-time
- Monday through Friday
- Possible on-call requirements
- Structured daily visit expectations
Key Responsibilities:
- Deliver skilled in-home nursing care and interventions to patients
- Complete initial and routine patient assessments with accurate OASIS documentation
- Develop and implement individualized care plans in collaboration with physicians and interdisciplinary teams
- Perform wound care, manage medications, provide patient and family education, and monitor disease progression
- Communicate changes in patient status to the clinical team and adjust care accordingly
Qualifications:
- Active and unencumbered RN license
- Minimum of 1 year recent experience as a Home Health RN
- Demonstrated proficiency in OASIS documentation
- HomeCare HomeBase experience is strongly preferred
- Reliable personal transportation and a valid drivers license
- Ability to independently manage a structured daily schedule with multiple patient visits
