Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be required
- Daily visit structure for effective caseload management

Responsibilities:
- Deliver skilled nursing care and interventions in a home setting
- Perform comprehensive patient assessments and complete timely, accurate OASIS documentation
- Collaborate closely with physicians and interdisciplinary care teams to develop and adjust individualized care plans
- Manage clinical tasks such as wound care, medication administration, disease monitoring, and patient/family education
- Communicate changes in patient conditions and care needs promptly to the clinical team
Qualifications:
- Active RN license
- Minimum of 1 year recent RN experience in home health
- Proficiency with OASIS documentation is required
- Familiarity with HomeCare HomeBase software is strongly preferred
- Capable of maintaining a structured daily schedule and visit expectations
- Valid drivers license and reliable transportation
