Your next nursing opportunity starts here. We are seeking a dedicated Registered Nurse with strong home health experience, including OASIS proficiency, for a full-time, 13-week contract assignment. This role is ideal for a skilled RN who excels in patient-centered care, detailed documentation, and thrives in an independent, structured schedule.

Schedule:

  • Full-time, Monday through Friday
  • Potential for on-call hours
  • Structured daily visit expectations

Key Responsibilities:

  • Deliver in-home skilled nursing care, including wound care, medication management, and disease monitoring
  • Perform comprehensive assessments and complete all OASIS documentation accurately and on time
  • Develop and implement individualized care plans in collaboration with physicians and interdisciplinary teams
  • Provide patient and family education to support health goals and safety
  • Communicate any patient status changes to the clinical team promptly

Qualifications:

  • Active RN license (unrestricted)
  • At least 1 year of recent home health RN experience
  • OASIS proficiency is required
  • Experience with HomeCare HomeBase is strongly preferred
  • Ability to manage a full-time schedule with daily patient visits
  • Reliable transportation and valid drivers license
