Job Description

Embark on a meaningful nursing role delivering compassionate care directly to patients in their homes. This full-time, 13-week contract position focuses on supporting individuals throughout Greensboro with comprehensive home health services.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

What Youll Do:

  • Conduct thorough OASIS assessments and develop personalized care plans tailored to each patients unique needs.
  • Administer skilled nursing interventions such as wound care, IV therapy, medication administration, and managing chronic conditions.
  • Monitor and evaluate patient progress, identifying changes in condition and communicating promptly with physicians and the interdisciplinary team.
  • Provide vital patient and family education focusing on disease management, medication understanding, and preventive strategies.
  • Accurately document all patient interactions and care using the Home Care Home Base (HCHB) electronic system.
  • Collaborate effectively with therapists, social workers, and home health aides to ensure an integrated approach to patient care.
  • Attend case conferences, staff meetings, and maintain ongoing professional education.
  • Uphold rigorous standards for patient care, regulatory compliance, and clinical documentation.

What Were Looking For:

  • Active, unrestricted RN license in North Carolina
  • At least one year of recent home health nursing experience, with field work preferred
  • Proficiency in Home Care Home Base (HCHB) and OASIS documentation
  • Strong clinical skills in home health settings
  • Reliable transportation, valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
