Your next nursing opportunity starts here. We are seeking a dedicated and experienced Registered Nurse (RN) with a background in home health and OASIS documentation for a 13-week full-time contract. This role allows you to use your clinical expertise where it matters mostdirectly in patients homesensuring continuity of care and meaningful, one-on-one connections.

Schedule:

• 13-week assignment with potential for extension
• Full-time, Monday through Friday
• On-call responsibilities may be required

Key Responsibilities:

• Deliver direct in-home nursing care and skilled interventions tailored to each patient
• Complete comprehensive patient assessments and timely, accurate OASIS documentation
• Collaborate with physicians and multidisciplinary care teams to establish individualized care plans
• Provide wound care, manage medications, conduct disease monitoring, and educate patients and families
• Communicate patient status updates and any changes in condition to the clinical team

Qualifications:

• Active RN license
• At least 1 year of recent home health RN experience
• Proficiency with OASIS documentation (required)
• Experience with HomeCare HomeBase strongly preferred
• Reliable transportation and a valid drivers license
• Excellent organizational and time management skills
