Your next nursing opportunity starts here! As a skilled Registered Nurse with home health experience, youll provide crucial care to patients in their homes over a 13-week full-time contract. This position is designed for an RN who excels in clinical assessment, thorough documentation, and maintaining a structured caseload. Schedule:
- 13-week full-time contract
- Monday through Friday, full-time hours
- On-call responsibilities likely
Key Responsibilities:
- Deliver compassionate in-home skilled nursing care, including patient assessments and disease monitoring
- Complete OASIS documentation promptly and accurately to support individualized care plans
- Collaborate closely with physicians and multidisciplinary care teams to ensure high-quality patient outcomes
- Provide wound care, medication administration, patient education, and support for chronic disease management
- Communicate patient progress, concerns, and changes to the clinical team in a timely manner
Qualifications:
- Current, active RN license
- At least 1 year of recent RN experience in home health
- OASIS documentation expertise required
- Proficiency with HomeCare HomeBase strongly preferred
- Ability to manage daily patient visits independently within a full-time caseload
- Reliable personal transportation and a valid drivers license
