This opportunity is tailored for a skilled Registered Nurse with home health and travel experience, ready to manage patient care within the Greensboro and High Point areas. As a travel home health RN, you will be responsible for maintaining and overseeing a caseload, ensuring quality care in patients' homes. This role requires a commitment to a contract period of 8 weeks, with potential extension to 12 weeks based on census needs.
Responsibilities:
- Manage and coordinate home health patient care plans within the designated service area
- Conduct patient visits, assessments, and documentation in accordance with home health standards
- Collaborate with interdisciplinary teams to optimize patient outcomes
- Provide case management, including prioritizing care and communicating with patients and families
- Travel throughout Greensboro and High Point to visit patients, using own transportation
- Maintain compliance with all regulatory and agency policies
Qualifications & Desired Experience:
- Current RN license with home health nursing experience
- Minimum of 1 year of case management experience in a home health setting
- Previous travel nursing experience preferred
- Ability to manage a patient caseload independently
- Must have reliable personal transportation and valid drivers license
- Willingness to travel within the assigned service area throughout the day
Schedule/Hours:
- Monday through Friday, 8:00 AM to 5:00 PM (5 days a week, 8-hour shifts)
