Job Description
Deliver essential nursing care in patients homes as a Home Health Registered Nurse, providing critical support and wellness management throughout the Greensboro/High Point area. This contract opportunity spans 8 weeks, with the potential for extension based on patient census.
Schedule:
- Monday to Friday, 8:00 AM to 5:00 PM
- Five 8-hour day shifts per week
- Day travel throughout Greensboro and High Point
- Mileage reimbursement provided at the federal rate
Qualifications:
- Active and unrestricted RN license
- Minimum of one year of case management experience is required
- Prior experience in Home Health and with HCHB (Homecare Homebase) documentation
- Previous travel nurse experience strongly preferred
- Reliable personal vehicle and willingness to travel extensively during each workday
- Exceptional organizational and time management skills
- Strong communication abilities and patient-focused approach
Key Responsibilities:
- Assume a full patient case load and provide comprehensive nursing care in a home setting
- Conduct thorough assessments, develop care plans, and evaluate ongoing patient needs
- Collaborate with interdisciplinary teams, patients, and families to promote optimal outcomes
- Accurately document patient interactions and interventions using Homecare Homebase
- Actively educate and support patients and caregivers in managing health conditions
- Prioritize visits and coordinate care delivery effectively across multiple locations each day
