Your next nursing opportunity starts here! We are seeking a dedicated Registered Nurse (RN) with strong OASIS experience for a 13-week home health assignment. This full-time contract is ideal for a skilled clinician who thrives in a structured environment and is passionate about delivering high-quality patient care in the home setting.
Schedule:
- 13-week full-time contract
- Monday through Friday schedule
- On-call availability may be required
Key Responsibilities:
- Deliver compassionate in-home nursing care, including skilled interventions and monitoring
- Conduct thorough patient assessments and complete OASIS documentation with accuracy and timeliness
- Collaborate closely with physicians and multidisciplinary care teams to create individualized care plans
- Provide wound care, medication administration, patient education, and disease management support
- Communicate effectively with clinical team members regarding patient status and changes
Qualifications:
- Active and unrestricted RN license
- At least 1 year of recent RN experience in home health
- Strong OASIS documentation experience required
- Proficiency with HomeCare HomeBase (highly preferred)
- Ability to efficiently manage a full-time, structured caseload with daily visits
- Reliable transportation and a valid drivers license
