We are seeking a dedicated Registered Nurse (RN) to provide skilled home health services on a full-time, 13-week contract. This position is ideal for someone with OASIS experience, exceptional clinical judgment, and confidence managing a schedule of daily visits.
Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be required
Key Responsibilities:
- Deliver evidence-based nursing care and interventions directly in patient homes
- Perform comprehensive assessments and complete OASIS documentation accurately and promptly
- Collaborate with physicians and interdisciplinary teams to develop individualized care plans
- Conduct wound care, medication administration, patient education, and ongoing disease management
- Communicate effectively with the clinical team regarding patient status and care updates
Qualifications:
- Active RN license in good standing
- At least one year of recent experience as a home health RN
- Strong proficiency in OASIS documentation (required)
- Familiarity with HomeCare HomeBase is highly preferred
- Ability to independently manage a full daily patient caseload
- Reliable transportation and a valid drivers license
