This full-time, 13-week home health contract is ideal for professionals who excel in clinical assessment, thrive in autonomous settings, and value meticulous documentationparticularly those with strong OASIS experience.

Schedule & Hours:

  • 13-week assignment, with potential for extension
  • Full-time, Monday through Friday
  • On-call responsibilities likely
  • Daily visit expectations provided in advance

Core Responsibilities:

  • Provide skilled nursing care and in-home interventions for patients
  • Perform patient assessments, ensuring all OASIS documentation is completed accurately and promptly
  • Coordinate with physicians and allied care providers to develop and implement personalized care plans
  • Deliver wound care, manage medications, educate patients and families, and monitor disease progression
  • Communicate clinical updates and changes in patient status to the care team

Qualifications:

  • Current, active RN license
  • At least one year of recent RN experience in home health required
  • Proficiency with OASIS documentation
  • Prior experience with HomeCare HomeBase is highly preferred
  • Able to effectively manage a structured, full-time schedule and complete daily visits
  • Must have reliable transportation and a valid drivers license
