Key Responsibilities:

  • Deliver in-home nursing care and skilled interventions for a variety of patient needs
  • Complete patient assessments and ensure accurate, timely OASIS documentation
  • Collaborate with medical professionals to create and follow personalized care plans
  • Provide wound care, medication administration, patient education, and chronic disease management
  • Communicate changes in patient status proactively to the clinical team

What Were Looking For:

  • Active RN license
  • At least 1 year of recent experience in home health nursing
  • Proven OASIS documentation expertise
  • Strongly preferred: proficiency with HomeCare HomeBase software
  • Ability to manage a full daily schedule of patient visits
  • Reliable transportation and a valid drivers license
