Step into a dynamic field-based nursing role with a focus on quality patient care in the comfort of patients homes. This full-time, 13-week assignment offers the opportunity to utilize your nursing expertise and make a meaningful impact through direct patient interaction.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based; daily travel to patient homes (mileage reimbursement provided)
• Caseload expectation: 30 points per week (typically 35 patient visits per day)

Key Responsibilities:

• Conduct thorough OASIS assessments and develop individualized care plans
• Deliver skilled nursing care such as wound management, medication administration, IV therapy, and chronic disease monitoring
• Continuously evaluate patient progress, recognizing and reporting any changes in condition
• Provide patient and family education on disease management, medications, and preventive health
• Ensure timely and accurate documentation using Home Care Home Base (HCHB)
• Collaborate effectively with interdisciplinary teamsPT, OT, social workers, and aidesfor comprehensive care delivery
• Participate in case conferences, staff meetings, and ongoing professional development
• Maintain adherence to all regulatory and agency documentation and care standards

Qualifications:

• Active RN license (in good standing) in the state of practice
• At least 1 year of recent, relevant Home Health experience (field experience preferred)
• Proficiency with Home Care Home Base (HCHB) and OASIS documentation required
• Valid drivers license, reliable transportation, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
