Were searching for an enthusiastic Registered Nurse with substantial home health experience and OASIS proficiency for a 13-week full-time contract, specializing in providing skilled care to patients in their own homes. This role is well-suited for RNs who are detail-oriented, confident in clinical decision-making, and thrive in a dynamic, autonomous environment.
Schedule:
- 13-week contract with potential for extension
- Full-time, Monday through Friday schedule
- On-call rotation likely
- Clear daily visit expectations
Primary Responsibilities:
- Deliver compassionate, in-home nursing care and skilled clinical interventions
- Accurately conduct patient assessments and complete timely OASIS documentation
- Coordinate closely with physicians and interdisciplinary teams to execute tailored care plans
- Perform wound care, medication management, education on disease processes, and other nursing duties
- Effectively communicate changes in patient condition and updates to the clinical team
Qualifications:
- Active RN license in good standing
- Minimum of 1 year recent experience as a home health RN
- Required OASIS documentation experience
- Proficiency with HomeCare HomeBase strongly preferred
- Demonstrated ability to manage full-time, structured caseloads with daily visits
- Reliable transportation and a valid drivers license
