Your next nursing opportunity is here: a full-time home health position for a Registered Nurse (RN) with OASIS experience, available for a 13-week contract. This is a chance to deliver skilled in-home care while collaborating with a knowledgeable, professional care team. Schedule:
- 13-week contract with potential to extend
- Monday through Friday, full-time hours
- On-call duties may be required
Responsibilities:
- Deliver in-home nursing care, perform skilled interventions, and ensure patient safety
- Conduct thorough patient assessments, completing OASIS documentation promptly and accurately
- Develop and implement individualized care plans alongside physicians and other clinicians
- Provide wound care, medication administration, disease management, and patient/family education
- Report changes in patient condition and collaborate with the clinical team for optimal outcomes
Qualifications:
- Active Registered Nurse license
- Minimum one year of recent home health RN experience
- Demonstrated OASIS experience (required)
- Proficiency with HomeCare HomeBase (strongly preferred)
- Reliable, organized, and able to manage full-time patient visits
- Valid drivers license and dependable transportation
