Schedule:
- 13-week contract with potential for extension
- Full-time: Monday through Friday

Key Responsibilities:
- Provide comprehensive in-home nursing care and skilled interventions
- Complete timely and accurate patient assessments, including OASIS documentation
- Collaborate with physicians and interdisciplinary care teams to create tailored care plans
- Perform wound care, medication management, disease monitoring, and patient/family education
- Communicate changes in patient condition and progress to the clinical team
Qualifications:
- Active RN license
- Minimum of 1 year of recent RN experience in home health
- Required: OASIS documentation proficiency
- Strongly preferred: Familiarity with HomeCare HomeBase
- Demonstrated ability to manage a structured caseload and adhere to daily visit expectations
- Reliable transportation and valid drivers license
