We are seeking a skilled Registered Nurse (RN) with solid home health experience, particularly with OASIS documentation, for a 13-week full-time contract. This is an outstanding opportunity to support individuals in their homes while collaborating with a dedicated and professional care team.
Schedule:
- 13-week contract, potential for extension
- Full-time, Monday through Friday
- Likely includes on-call responsibilities
- Structured daily visit expectations
Qualifications:
- Current, active RN license
- Minimum of 1 year of recent home health RN experience
- Proficiency in OASIS required
- Experience with HomeCare HomeBase (strongly preferred)
- Strong documentation and clinical assessment skills
- Valid drivers license and reliable transportation
- Demonstrated ability to manage a set visit schedule
Responsibilities:
- Deliver in-home nursing care, interventions, and patient education
- Complete patient assessments and timely, accurate OASIS documentation
- Collaborate with physicians and interdisciplinary teams to develop and implement care plans
- Conduct wound care, medication management, and monitor ongoing health needs
- Provide timely clinical updates to team members and respond to changes in patient condition
