Your next nursing opportunity starts here. We are seeking a skilled Registered Nurse (RN) with proven OASIS experience for a 13-week full-time contract in home health. Schedule:
- Full-time: Monday through Friday
- On-call responsibilities may be required
- 13-week contract with extension potential
- Structured daily visit expectations
Key Responsibilities:
- Deliver in-home nursing care and skilled interventions tailored to each patient
- Conduct comprehensive assessments and complete OASIS documentation accurately and punctually
- Collaborate with physicians and multidisciplinary teams to implement individualized care plans
- Perform wound care, medication management, patient and caregiver education, and disease monitoring
- Ensure timely communication of patient status and any changes to the clinical team
Qualifications:
- Current and active RN license
- Minimum 1 year of recent home health RN experience
- Demonstrated proficiency with OASIS documentation
- Experience using HomeCare HomeBase (strongly preferred)
- Ability to handle a structured schedule with daily visits
- Reliable vehicle and a valid drivers license
