Job Description

A skilled and compassionate Registered Nurse (RN) with home health experience is needed for a contract assignment supporting patients in a home-based care setting. This position is ideal for an RN with strong clinical assessment skills, OASIS experience, and a commitment to delivering high-quality patient care outside the traditional hospital environment.

Schedule:

• Contract position
• Flexible scheduling may be available based on assignment needs

Qualifications:

• Active, unrestricted RN license
• At least 3 months of recent home health experience using OASIS mandatory
• Familiarity with HCHB (Homecare Homebase) is highly desirable
• Proficient clinical skills, including IV therapy, wound care, and medication management
• Confidence in using EMR systems for documentation
• Outstanding communication and interpersonal skills
• Ability to work autonomously and maintain effective time management
• Valid drivers license and access to reliable transportation

Primary Responsibilities:

• Deliver comprehensive nursing care to patients in their residences
• Conduct initial and follow-up assessments, monitor vital signs, and oversee medication administration
• Develop, implement, and update care plans tailored to each patient
• Provide education to patients and their families on disease management and preventive care
• Coordinate care and collaborate with physicians, therapists, and other healthcare professionals
• Document interactions and care promptly and accurately in the EMR
• Recognize and communicate changes in patient condition to the supervising physician
• Ensure all care complies with current home health regulations and best practices
