Schedule:
- Full-time, Monday through Friday
- Consistent daily visit expectations
- Potential for on-call responsibilities
Responsibilities:
- Deliver skilled in-home nursing care and essential interventions to a diverse caseload
- Conduct thorough patient assessments and complete OASIS documentation with accuracy and timeliness
- Collaborate closely with physicians and interdisciplinary teams to ensure individualized, high-quality care
- Perform wound care, manage medications, provide patient and family education, and oversee disease monitoring
- Communicate patient status changes effectively with the clinical team to ensure coordinated, responsive care
What you bring:
- Active Registered Nurse license
- Minimum 1 year of recent home health RN experience
- Demonstrated proficiency in OASIS documentation
- Strong preference for experience with HomeCare HomeBase software
- Skill in managing structured schedules and balancing daily visit loads
- Reliable transportation and valid drivers license
