We're seeking a dedicated Registered Nurse with a background in home health and OASIS documentation for a rewarding 13-week full-time contract. If youre confident in delivering high-quality care, excelling in clinical documentation, and managing a full-time caseload, this could be an ideal fit.

Schedule:

  • Full-time, Monday through Friday
  • On-call rotation may be required

Key Responsibilities:

  • Deliver skilled, evidence-based nursing care directly in patients homes
  • Accurately conduct patient assessments and consistently complete OASIS documentation within required timelines
  • Partner with physicians and interdisciplinary teams to establish and follow individualized care plans
  • Provide wound care, manage medications, deliver patient and family education, and monitor for changes in health status
  • Communicate promptly with clinical teams regarding updates or concerns with patient conditions

Who Were Looking For:

  • Active RN license
  • At least 1 year of recent RN experience in home health
  • Demonstrated proficiency with OASIS documentation (required)
  • Experience with HomeCare HomeBase (strongly preferred)
  • Strong organizational skills and ability to handle structured daily visits
  • Reliable transportation and a valid drivers license
