An opening is available for an experienced Registered Nurse (RN) skilled in home health care, specifically with OASIS expertise, for a 13-week full-time contract. In this role, you will have the chance to support patients in their homes and impact lives through outstanding clinical care and accurate documentation.

Schedule:

  • 13-week contract with the potential for extension
  • Full-time, Monday through Friday
  • On-call hours likely as part of the rotation
  • Structured daily visit expectations

Key Responsibilities:

  • Provide comprehensive in-home nursing care and skilled clinical interventions
  • Complete thorough patient assessments, ensuring precise and timely OASIS documentation
  • Collaborate closely with physicians and multidisciplinary care teams
  • Deliver wound care, medication management, patient and family education, and disease monitoring
  • Communicate promptly regarding changes in patient condition to appropriate members of the clinical team

Qualifications:

  • Active and unrestricted RN license
  • Minimum of one year recent experience as a home health RN
  • Proven experience with OASIS documentation
  • Proficiency with HomeCare HomeBase is strongly preferred
  • Reliable transportation and a valid drivers license are required
  • Confident managing a structured full-time schedule and daily patient caseload
