Advance your nursing career with a rewarding full-time, 13-week contract opportunity providing essential home health services in a field-based setting. This role focuses on delivering compassionate, high-quality care to patients in their homes, ensuring both clinical excellence and meaningful patient relationships.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Caseload:

• 30 points per week (generally 35 patient visits per day)

Responsibilities Include:

• Conduct comprehensive OASIS assessments and develop customized care plans
• Deliver skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management
• Assess patient progress, recognize health status changes, and communicate effectively with the broader care team and physicians
• Educate patients and families on diagnosis, care plans, medications, and preventive health strategies
• Complete timely and accurate electronic documentation using Home Care Home Base (HCHB)
• Collaborate closely with multidisciplinary team members, including therapists, medical social workers, and home health aides
• Participate in regular case conferences, staff meetings, and continuing education sessions
• Remain compliant with all regulatory and agency documentation and patient care standards

Qualifications:

• Active RN license in the state of practice, in good standing
• At least 1 year of recent field-based Home Health experience
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments
• Reliable transportation, valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
