As a Registered Nurse providing home health care, youll expand your skill set while delivering compassionate, patient-centered care in a variety of home settings. Each patient presents unique challenges, empowering you to think critically and adapt your clinical approach. Working closely with physicians, home health aides, and licensed practical/vocational nurses, youll deepen your understanding of effective supervision and multidisciplinary care.
Schedule/Hours:
- Travel between patient homes within the area
- Participation in periodic office meetings
Key Qualifications:
- Current, unencumbered RN license (specific to the assigned state)
- Minimum of 2-3 years of nursing experience
- Graduate of an accredited professional nursing program (BSN preferred)
- Current CPR certification
- Reliable transportation, valid drivers license, and liability insurance
- Strong assessment skills and ability to address patient-specific needs
- Comfortable with electronic charting and digital communication
Responsibilities:
- Deliver home nursing care as directed by physicians and according to agency policies
- Conduct initial and ongoing assessments, documenting findings and interventions
- Develop individualized care plans, revisiting and updating as patient needs evolve
- Supervise and evaluate home health aides and LPNs/LVNs, ensuring care quality
- Communicate promptly with physicians about changes in patient conditions
- Maintain accurate patient records and progress notes
- Provide education and counseling to patients and families
- Participate in staff development programs and continuous learning
