Take your nursing expertise into the community by providing impactful care in a home health setting. This full-time, 13-week contract position is focused on delivering high-quality, patient-centered nursing to individuals in their own homes. Schedule & Structure:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based role (home visits)
• Caseload of approximately 35 patient visits per day (30 points per week)

What Youll Be Doing:

• Complete in-depth OASIS assessments and develop individualized patient care plans.
• Deliver skilled nursing interventions: wound care, IV therapy, medication administration, chronic disease management, and more.
• Monitor patient progress, recognize changes in condition, and communicate updates promptly with physicians and the care team.
• Provide thorough education to patients and families regarding treatment plans, medication regimens, and preventive health strategies.
• Accurately document all care and interactions using Home Care Home Base (HCHB).
• Coordinate seamlessly with physical therapists, occupational therapists, medical social workers, and home health aides to ensure comprehensive care.
• Attend case conferences and staff meetings, and participate in ongoing education as required.
• Maintain high standards of compliance with agency and regulatory documentation and care requirements.

What Youll Need:

• Active RN license (state of practice) in good standing
• Minimum 1 year of recent home health field experience
• Proficiency with HCHB and OASIS documentation
• Reliable transportation, valid drivers license, and proof of auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
