Your next nursing opportunity awaitsa dynamic role as a Registered Nurse providing essential home health services on a 13-week contract. This position is ideal for RNs who excel in clinical documentation, possess strong organizational skills, and are passionate about making a genuine difference in patients lives.
Schedule:
- Full-time, Monday through Friday
- On-call responsibility may be required
- Structured daily visit expectations
Key Responsibilities:
- Deliver skilled in-home nursing care and specialized interventions
- Complete accurate and timely patient assessments and OASIS documentation
- Coordinate with physicians and interdisciplinary teams to create and implement individualized care plans
- Perform wound care, medication administration, patient education, and chronic condition monitoring
- Communicate changes in patient condition effectively with the clinical team
Qualifications:
- Active RN license
- Minimum of one years recent experience in home health nursing
- Proficient with OASIS documentation (required)
- Strongly prefer experience with HomeCare HomeBase
- Reliable transportation and a valid drivers license
- Comfortable with managing a full-time, structured schedule and multiple daily visits
