An opportunity is available for an experienced Home Health Registered Nurse (RN) to deliver skilled nursing care within patient homes. This role requires a compassionate professional who can work independently while upholding a high standard of clinical excellence and strong patient communication.
Responsibilities include conducting comprehensive OASIS assessments, developing individualized care plans, and providing skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management. The RN will monitor patient progress, communicate changes to the care team, educate patients and families, and maintain accurate documentation using Home Care Home Base (HCHB). Collaboration with therapists, social workers, and home health aides to ensure coordinated care is essential. Adherence to regulatory and agency standards is mandatory.
This full-time contract position runs for 13 weeks, with a Monday to Friday schedule from 8:00 AM to 5:00 PM. Caseload expectations range between 2532 points weekly, averaging 35 patient visits per day. The position involves daily travel within an assigned territory, with mileage reimbursement provided.
Qualifications:
- Active RN license in good standing within the state of practice
- Minimum one year of recent Home Health experience (field experience preferred)
- Proficiency in Home Care Home Base (HCHB) and OASIS required
- Strong clinical assessment and critical thinking skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently and manage a flexible field-based schedule
- Excellent communication, time management, and organizational abilities
Preferred:
- BSN degree
- Experience managing 2532 points per week
- Knowledge of Medicare and home health compliance standards
