Schedule:

  • Full-time, Monday through Friday
  • Potential for on-call responsibilities
  • Structured daily patient visit expectations

Qualifications:

  • Active RN license in good standing
  • Minimum one year of recent experience as a Home Health RN
  • OASIS documentation proficiency required
  • Experience with HomeCare HomeBase (highly preferred)
  • Reliable transportation and valid drivers license
  • Ability to manage daily home visits independently

Responsibilities:

  • Deliver skilled nursing and personalized care to patients in their homes
  • Conduct thorough patient assessments and ensure timely, accurate completion of OASIS documentation
  • Develop, implement, and coordinate individualized care plans in collaboration with physicians and interdisciplinary team members
  • Provide wound care, medication management, education, and monitor chronic disease progression
  • Communicate effectively regarding changes in patient condition
