We're seeking a dedicated Registered Nurse (RN) with a strong background in home health and proven OASIS documentation skills for a 13-week contract. This role is designed for detail-oriented professionals who thrive in autonomous settings and are confident in managing structured patient schedules.
Schedule:
- Full-time, Monday through Friday
- Potential for on-call responsibilities
- Structured daily patient visit expectations
Qualifications:
- Active RN license in good standing
- Minimum one year of recent experience as a Home Health RN
- OASIS documentation proficiency required
- Experience with HomeCare HomeBase (highly preferred)
- Reliable transportation and valid drivers license
- Ability to manage daily home visits independently
Responsibilities:
- Deliver skilled nursing and personalized care to patients in their homes
- Conduct thorough patient assessments and ensure timely, accurate completion of OASIS documentation
- Develop, implement, and coordinate individualized care plans in collaboration with physicians and interdisciplinary team members
- Provide wound care, medication management, education, and monitor chronic disease progression
- Communicate effectively regarding changes in patient condition
