Your next nursing opportunity starts here. We are seeking a dedicated Registered Nurse (RN) with OASIS expertise to provide crucial home health care on a 13-week full-time contract. Schedule:

• 13-week contract with potential for extension
• Full-time, Monday through Friday
• On-call duty may be required
• Structured daily visit expectations

Key Responsibilities:

• Deliver skilled nursing care to patients in their homes, including assessments, interventions, and follow-ups
• Complete OASIS documentation accurately and in accordance with deadlines
• Collaborate with physicians and interdisciplinary care teams to develop and implement personalized care plans
• Perform wound care, administer medications, monitor chronic conditions, and educate patients and families on disease management
• Communicate changes in patient condition and care updates promptly to the clinical team

Qualifications:

• Active RN license
• Minimum of 1 year of recent experience in home health nursing
• Proficient in OASIS documentation (required)
• Familiarity with HomeCare HomeBase (strongly preferred)
• Organized and able to manage a consistent, full-time schedule with daily patient visits
• Possesses reliable transportation and valid drivers license
