This 13-week, full-time contract opportunity is ideal for experienced registered nurses seeking a dynamic role in home health care. The position is field-based, providing skilled nursing visits to patients in their homes within a defined territory.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

Role Expectations:

• Manage a caseload of approximately 30 points per week (typically 35 patient visits per day)
• Complete daily travel to patient homes (mileage reimbursement provided)

Key Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans for home health patients
• Deliver skilled nursing care such as wound management, IV therapy, chronic disease management, and medication administration
• Monitor patient progress, assess changes in condition, and communicate timely updates to the care team and physicians
• Educate patients and families about disease processes, medication regimens, treatment plans, and preventive care
• Accurately and promptly document all care provided using Home Care Home Base (HCHB)
• Coordinate care with therapy staff, social workers, and home health aides to ensure patient-centered service
• Attend case conferences, staff meetings, and participate in ongoing education as necessary
• Comply with all regulatory and agency documentation standards

Qualifications:

• Active RN license in good standing for the state of practice
• Minimum one year of recent field-based home health experience
• Proficiency with Home Care Home Base (HCHB) and OASIS required
• Reliable transportation, valid drivers license, and auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
