Schedule:
- Full-time, Monday through Friday
- On-call coverage may be required
- Daily visit expectations
What Youll Do:
- Provide comprehensive in-home nursing care, including assessments and skilled interventions
- Complete timely and accurate OASIS documentation for all patient visits
- Collaborate with physicians and interdisciplinary teams to implement individualized patient care plans
- Deliver wound care, medication management, patient education, and ongoing disease monitoring
- Communicate changes in patient status promptly to the clinical and office teams
Qualifications:
- Current and active RN license
- At least 1 year of recent home health experience as an RN
- OASIS assessment and documentation experience required
- Proficiency with HomeCare HomeBase (strongly preferred)
- Reliable transportation and valid drivers license
- Ability to independently manage a structured, full-time workload
