We are searching for a skilled Registered Nurse (RN) who excels in clinical care, accurate documentation, and managing a set daily schedule. Schedule & Duration:

• 13-week contract with possible extension
• Full-time, Monday through Friday
• On-call duties may be required

Key Responsibilities:

• Deliver in-home nursing care and skilled health interventions
• Complete comprehensive patient assessments and OASIS documentation promptly
• Collaborate with physicians and multidisciplinary teams to develop and implement individualized care plans
• Administer wound care, manage medications, provide patient education, and monitor disease status
• Communicate all patient updates and changes in condition to the clinical team in a timely manner

Qualifications:

• Active RN license
• Minimum of 1 year recent experience as a home health RN
• Proficiency in OASIS is essential
• Experience with HomeCare HomeBase (strongly preferred)
• Reliable transportation and a valid drivers license
• Ability to independently manage a structured, full-time schedule with daily patient visits
