A highly regarded care team is seeking an experienced Registered Nurse (RN) with strong OASIS proficiency for a full-time, 13-week home health contract. This role is ideal for a clinically confident RN skilled in accurate documentation and dedicated to providing outstanding in-home care.
Schedule:
- 13-week contract, full-time position
- Monday through Friday
- On-call may be required
Key Responsibilities:
- Deliver safe, individualized nursing care to patients in their homes
- Conduct thorough patient assessments and complete timely, accurate OASIS documentation
- Collaborate with providers and interdisciplinary teams to implement personalized care plans
- Perform wound care, medication management, patient education, and monitor chronic conditions
- Provide regular communication and clinical updates on patient status
Qualifications:
- Active RN license in good standing
- At least 1 year of recent experience as a home health RN
- Proficiency with OASIS documentation (required)
- Experience with HomeCare HomeBase is strongly preferred
- Ability to effectively organize daily visits and manage a full patient caseload
- Reliable transportation and a valid drivers license
