Advance your nursing career by providing compassionate, high-quality care directly to patients in a field-based home health setting. This is a full-time, 13-week contract supporting individuals who rely on your expertise and dedication throughout their recovery at home.

Schedule and Hours:

• Monday Friday, 8:00 AM 5:00 PM

Role Overview: As a registered nurse in this home health assignment, youll perform 35 patient visits per day, aiming for a manageable caseload of 30 points per week. Daily travel within your assigned territory is required, with mileage reimbursement provided. Your role focuses on comprehensive, patient-centered care using the Home Care Home Base (HCHB) electronic documentation system and conducting OASIS assessments.

Essential Qualifications:

• Active RN License in good standing for the state
• Minimum 1 year of recent field home health nursing experience
• Proficient in Home Care Home Base (HCHB) documentation
• Prior experience performing OASIS assessments
• Reliable transportation, current drivers license, and active auto insurance

Key Responsibilities:

• Complete in-depth OASIS assessments to tailor individualized care plans
• Deliver skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic condition management
• Monitor patient progress, communicate any changes to physicians and the care team
• Educate patients and their families on treatment regimens and preventive care strategies
• Document all care and communication in HCHB accurately and timely
• Collaborate with multidisciplinary team members to ensure cohesive, holistic care
• Participate in staff meetings, case conferences, and ongoing education

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
