An opportunity is available for a skilled Registered Nurse (RN) specializing in home health to provide essential care to patients in their residences. This contract role requires a professional committed to delivering tailored nursing care and collaborating closely with healthcare teams to optimize patient outcomes.
Key Responsibilities:
- Conduct comprehensive initial and ongoing patient assessments, including vital signs, medication administration, and wound care.
- Develop, implement, and update individualized care plans based on patient conditions and needs.
- Educate patients and families on disease management, treatment options, and preventive strategies.
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care.
- Accurately document patient interactions and care activities using electronic medical records (EMR).
- Monitor patient progress and promptly communicate any condition changes to supervising physicians.
- Ensure adherence to state and federal regulations governing home health care services.
Qualifications:
- Active Registered Nurse license.
- Minimum of 3 months experience in home health nursing with proficiency in OASIS assessments.
- Experience with Homecare Homebase (HCHB) software is highly desirable.
- Strong clinical competencies including intravenous therapy, wound management, and medication administration.
- Proficient with electronic medical record systems.
- Excellent communication, interpersonal skills, and ability to work independently.
- Valid drivers license and dependable transportation.
